Job summary

The post holder will develop and sustain relationships and networks with groups, individuals and agencies within a strategic framework. To act as a source of clinical knowledge and expertise to people with diabetes, their carers, health and other statutory and non‑statutory agencies, and to work as part of the Integrated Diabetes Service, in delivering services as directed. The post holder will be a Registered Nurse (RN12‑NMC) responsible for patients allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable.

Main duties of the job

To be responsible for organising and prioritising own and others workload in the day to day allocation of work. 2. To deputise when required in the team managers absence and delegate appropriately. 3. To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling. 4. To be responsible for providing accurate records of information required by the Trust for audit purposes. 5. To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register. Leadership 1. To participate in the development of an effective team and the development of productive working relationships throughout the Trust. 2 2. To actively promote integrated health professional working internally and externally. 3. To facilitate the development of a positive and supportive team culture by taking responsibility for dealing effectively with potential conflict.

Diabetes Specialist Nurse - North East London NHS Foundation Trust
We are seeking a dedicated and skilled Diabetes Specialist Nurse to join our experienced team in the Waltham Forest community. This permanent Band 7 role, based at the Jane Atkinson Centre, offers a salary of £49,397 - £56,515 (pro‑rated for part‑time hours). Working Monday to Friday, 9am‑5pm. This is a permanent role.

The Role
As a Diabetes Specialist Nurse, you will play an integral role in improving the quality of life for patients living with diabetes. Working within a highly experienced community diabetes team, you will provide patient education, lead service development, and collaborate with primary and secondary care colleagues to ensure the best possible outcomes. Your clinical expertise and leadership will contribute to managing a complex and autonomous caseload.

  • Provide tailored, structured education to empower patients and support self‑management.
  • Contribute to multidisciplinary team (MDT) meetings to develop holistic care plans.
  • Exhibit advanced clinical skills in managing a diverse, complex patient population.
Essential Skills and Qualifications

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop‑in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.

This post will be subject to a probationary period.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,870 and a maximum payment of £6,137 per annum, pro rata for part time)

High Cost Area Supplement - Fringe

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,346 and a maximum payment of £2,270 per annum, pro rata for part time)
