Diabetes Specialist Nurse

The Norfolk Community Health and Care NHS Trust Diabetes Team is primarily based in West Norfolk. We are a growing, friendly, and supportive team that provides care for people living with diabetes in various settings, including clinics, patients' homes, and care settings.

As part of an innovation, a fantastic opportunity has arisen to work within the NCHC Diabetes Team. We have one Band 6 position available in South place, 22.5 hours per week. You will work alongside a Band 4 Assistant Practitioner and under the supervision of the Diabetes Specialist Nurse Lead for NCHC.

The role requires professional and clinical experience. Due to the nature of the condition and evolving service changes, personal resilience is essential. Patients often have complex health needs requiring high levels of emotional, educational, and professional support. The team and wider organization provide a strong support network.

Main duties of the job
  1. Provide expert knowledge and skills within the Diabetes Team.
  2. Manage a caseload of patients with complex diabetes needs, using evidence-based, patient-centered approaches to assess, plan, implement, and evaluate interventions.
  3. Deliver health education to service users, their families, carers, and involved agencies, promoting self-management where possible.
  4. Work independently in patients' homes across your area, with remote access to the diabetes team.
  5. Hold a full UK driving license and have access to a vehicle for county-wide travel.
  6. Serve as an expert resource for primary healthcare teams supporting diabetes care in the community.
  7. Conduct holistic reviews of patients on the community insulin caseload.
  8. Act as a link nurse for nursing and residential homes, assisting with training.
  9. Provide advice and guidance to community nursing teams on diabetes management.
  10. Support patients in becoming independent with insulin administration and management, including those new to insulin therapy.
  11. Review new referrals to community insulin caseloads.
  12. Collaborate with other organizations.

The community diabetes service is a busy, nurse-led team working across West Norfolk to assess and manage patients with complex diabetes and co-morbidities.

We offer support and education to healthcare professionals and run structured education sessions for patients. The role offers flexible working options and opportunities for skill development and learning.

The successful candidate will have experience in managing diabetes and working with patients living with diabetes, with evidence of extended learning. Post-registration qualifications in diabetes are desirable.

Job responsibilities

Deliver a specialist care and support service in West Norfolk, including:

  1. Providing expert care and support in various settings, including home visits and telephone support.
  2. Offering educational support to professionals running diabetes clinics in general practice.
  3. Delivering a specialist diabetes service across community care, clinics, nursing/care homes, and housebound patients, complementing existing services.
  4. Promoting and raising the profile of diabetes management within local trusts, to professionals, patients, and families.
  5. Supporting improvements in standards of care aligned with the NSF for Diabetes and NICE guidelines.
Person Specification
Qualifications
  • Registration in adult nursing with extensive post-registration experience.
  • First Degree or working towards one.
Experience
  • Recent clinical experience in managing individuals with diabetes.
  • Experience in community care settings.
