Job description

We are seeking an enthusiastic and highly motivated individual to join Salford Integrated Diabetes Team as a Diabetes Health Adviser.

The purpose of this role is to work as part of the Diabetes Team, under the guidance of the specialists to provide structured education to patients with Type 2 Diabetes, promoting self‑management and education regarding the management of type 2 diabetes and supporting those patients with type 2 diabetes who are new to insulin, as well as be integral to the development of the type 2 diabetes education service.

This is a patient‑facing role and is suitable for someone who enjoys educating and caring for people. The candidate will be provided with support and training.

The successful candidate will be required to work at various locations within Salford, therefore a driving qualification is recommended.

Unfortunately, this role does not meet the criteria required to qualify for sponsorship.

Responsibilities
  • To support the education team within the Integrated Diabetes Team Department; working as an integral member of the team to facilitate the therapeutic needs of patients with type 2 diabetes who are under the care of the diabetes team.
  • Manage patients with type 2 diabetes within an agreed scope of practice as allocated by the supervising Dietitian. Working without direct supervision and undertaking a variety of delegated clinical and non‑clinical tasks, in accordance with agreed guidance/care pathways.
  • Provide a high standard of health care by assisting in and carrying out a range of individual and group education programs, in the community setting, for patients with type 2 diabetes, for which an agreed level of competency has been demonstrated under the direction of a Registered Dietitian.
  • Update resources for education programs and source new materials as needed.
  • Support and develop pathways for patient self‑referral utilising appropriate communication channels, networking with local community groups and developing relationships with GP practices and local pharmacies.
