Pontardawe Primary Care Centre is seeking an experienced Band 6 Practice Nurse to join our friendly and supportive team for 30 hours per week.

The role prioritises chronic disease reviews, with a focus on diabetes, plus wound care, immunisations and clinical audits. Baby clinics and cervical screening experience are desirable but not essential.

You will work collaboratively within a multidisciplinary team in a practice that values continuity of care and professional development, using EMIS Web.

Qualifications

• Experience running chronic disease clinics, including diabetes reviews.
• Wound assessment and management skills.
• Competence in routine vaccination administration and emergency procedures.
• Experience undertaking clinical audits and contributing to improvements.

Responsabilités

• Chronic disease reviews: run structured clinics, especially diabetes, with education and care planning.
• Clinical audit: identify improvement opportunities and implement changes.
• Wound care: assess and manage acute and chronic wounds and know when to escalate.
• Hypertension monitoring: conduct hypertension reviews and ambulatory BP monitoring with team support.
• Vaccinations: deliver routine and seasonal immunisations per training.
• General practice nursing: health promotion, preventive care and routine nursing interventions.
