We are seeking an experienced Band 6 Practice Nurse to join our warm, welcoming and supportive team at Pontardawe Primary Care Centre for 30 hours per week.
The initial priority for this role is delivering chronic disease reviews, with a particular focus on diabetes. Previous general practice nursing experience, including independently running chronic disease clinics and undertaking clinical audits, is essential. Strong wound-care skills and experience delivering routine and seasonal vaccinations are also required.
The role will develop in stages, with an initial emphasis on maintaining chronic disease services and continuity of care. Experience in baby clinics and cervical screening is desirable but not essential.
We offer a friendly working environment where colleagues support one another, professional expertise is valued and high standards of patient care are a shared responsibility.
Baby clinics and cervical screening may form part of the wider role where the successful candidate has the relevant training and competence.
At Pontardawe Primary Care Centre, our people and working environment are among our greatest strengths. We are a warm, friendly and welcoming practice, where teamwork and mutual support underpin the care we provide.
As part of the Upper Valleys cluster within Swansea Bay University Health Board, we serve approximately 13,300 patients. Our team comprises three GP partners, six salaried GPs, an Advanced Nurse Practitioner, a Physician Associate, 2 Paramedics, a pharmacy technician and pharmacy team, four practice nurses, three healthcare support workers, and a dedicated administrative team, supported by our Practice Business Manager. We are also a training practice and currently support 2 GP registrars.
We combine a commitment to high standards with a supportive culture that helps colleagues perform well. We value open communication, shared learning and practical support, and we actively seek to make the practice a good place to work.
We use EMIS Web, and experience with this clinical system would be desirable.
If you are an experienced practice nurse who values continuity of care, enjoys working collaboratively and wants to contribute to a friendly and supportive team, we would welcome your application.
