Responsibilities

• Operate nurse-led clinics to assess, diagnose, plan, implement and evaluate treatment for diabetes, pre-diabetes or undifferentiated cases.
• Collaborate with practice team to lead planning and delivery of diabetes services and promote integrated pathways of care.
• Initiate insulin and GLP-1s where indicated.
• Provide support and mentorship to colleagues within the practice.
• Contribute to local and national practice targets (e.g. QOF).
• Refer patients to other services/agencies as appropriate.
• Promote health through screening and health promotion initiatives.
• Undertake home visits for patients with diabetes when required.
• Maintain accurate clinical records in line with legislation and coding practices.
• Ensure appropriate use of chronic disease registers and monitor adherence and side effects of treatments.
• Request pathology services and process results as required.
• Liaise with external services to ensure patient support, including vulnerable patients.
• Delegate clinical responsibilities safely within scope of practice.
