Job summary

NHS Jobs is seeking a Practice Nurse with diabetes-focused expertise to deliver high-quality, evidence-based care in a general practice setting in Macclesfield. The role covers diabetes management, immunisations, wound care, women's health, and long-term condition reviews within a multidisciplinary team.

You will work autonomously within your scope, improving outcomes, supporting self-management and contributing to QOF targets. NMC registration required.

Qualifications

• Annual and interim reviews for Type 1 and Type 2 diabetes with interpretation of HbA1c, renal function, lipids and BP.
• Develop personalised care plans, education, lifestyle advice, self-management and glucose monitoring.
• Review and optimise diabetes medication per NICE guidance and local pathways.
• Wound care, dressings, sutures removal and leg ulcer care within competence.

Responsibilities

• Carry out diabetic foot assessments and risk stratification, and refer appropriately.
• Monitor for complications and escalate diabetes emergencies; refer to specialist services.
• Follow up non-attenders and liaise with community, specialist and secondary care teams.
• Provide immunisations and travel health where trained; maintain cold chain and cervical screening.
• Venepuncture, specimen collection, point-of-care testing, ECGs and injections; routine observations.
• Support minor surgery and GP-led procedures; provide aftercare and safety-netting.
