Our fabulous Urology Specialist Nursing Team are excited to offer this newly created Band 6 post. As part of the team, you will be supporting our patients on their Cancer Pathway journey – ensuring their emotional, psychological and physical health needs are met. Here at NGH we are fortunate to have a bespoke pathway and team for our Urology patients, with access and support from them to support your own journey. As part of the Macmillan family, you will have access to their resources.

Working as a core member of the Urology multidisciplinary team, the post holder will provide high‑quality, patient‑centred care, ensuring patients/carers living with prostate cancer have access to expert information, advice and support from the point of diagnosis. The post holder will act as the main point of contact, supporting a caseload of patients, liaising with other healthcare professionals within the MDT, primary and secondary care to ensure a seamless service for patients and their families.

Responsibilities
  • Support the Prostate Clinical Nurse Specialists in the delivery of an innovative, high‑quality service to those living with prostate cancer.
  • Implement the key elements of Personalised Care, ensuring patients are offered a Holistic Needs Assessment/care planning at key stages in the pathway and an End of Treatment Summary and Health and Wellbeing opportunities.
  • Work across both Kettering and Northampton hospital sites, where required.
  • Undertake a HNA at key stages in the pathway using a recognised tool, ensuring all patient needs are identified.
  • Use supportive conversations to develop and implement plans of care to meet the individual needs of patients/carers, reviewing and adapting as their clinical situation changes.
  • Attend the weekly specialist multidisciplinary team meeting, contributing to discussion and patient assessment/care planning decisions.
  • Review patients admitted as an emergency, working with the admitting team to agree and implement an appropriate management plan.
  • Provide advice on symptom management, referring those with complex needs to the Macmillan Specialist Palliative Care Team.
  • Demonstrate enhanced practice in planning, implementing and evaluating nursing care, documenting outcomes in hospital notes and on the Somerset Cancer Register.
  • Provide Level 2 psychological support for patients/carers and refer those with complex needs to the Macmillan Clinical Psychologist.
  • Support patient and carer communication issues and coordinate the patient's pathway, acting as point of contact or nominating the main point of contact.
  • Ensure patients receive specialist verbal and written information at the appropriate point in their illness experience, signposting to the Macmillan Information Centre and Maggie’s.
  • Become acquainted with local and regional care pathways, including knowledge of contact points for referral and advice.
  • Identify and meet the educational needs of those living with prostate cancer.
  • Demonstrate knowledge of the clinical trials relevant to prostate cancer.
  • Ensure palliative care needs are identified and met, referring patients with complex needs to the Macmillan Specialist Palliative Care Team.
  • Identify the patient's Preferred Place of Care, ensuring patients can be discharged to their place of choice where possible.
  • Support patients under the AMBER Care Bundle pathway.
  • Ensure patients/carers are supported during significant consultations, acting as the patient's advocate.
  • Promote a culture that supports self‑management strategies and the dedicated cancer You‑Tube channel.
  • Work in line with the NMC Code of Professional Conduct.
  • Promote collaborative working relationships and effective communication between all members of the multidisciplinary team and across care boundaries.
  • Provide nursing advice and support to other health professionals related to prostate cancer.
  • Identify own personal development needs and ensure they are met, engaging with learning and development opportunities offered by Macmillan Cancer Support.
  • Maintain a personal professional profile, demonstrating evidence for revalidation.
  • Participate in the formal review process and plan future personal development and competencies to be achieved.
