Job summary
Norfolk and Norwich University Hospitals NHS Foundation Trust is seeking a Dementia Specialist Practitioner to deliver specialist assessment, care planning, and person-centred interventions for in-patients with dementia. - Strong collaboration with multi-disciplinary teams to optimise patient outcomes.
Responsibilities
- Provide specialist dementia assessments and care planning for complex cases.
- Prioritise caseload and coordinate with Dementia Support Workers, families and staff.
- Promote person-centred care and interventions to maintain skills and engagement in care.
- Work independently and with the dementia support team across hospital wards.
- Provide regular supervision and participate in inductions and ongoing training.
