NHS Forth Valley seeks a Dementia, Delirium Consultant Nurse/AHP to lead and provide expert consultancy on care for patients with Dementia and Delirium. NHS Forth Valley is one of 14 regional health boards in Scotland and serves a population of nearly 306,000 in a diverse geographical area which covers the heart of Scotland and covers three council areas, CClackmannanshire, Stirling and Falkirk. We provide a range of primary, community based and acute hospital services and have strong strategic partnerships with our two Integration Joint Boards, three Local Authorities and our local University and College. An exciting opportunity has arisen for an engaging and inspiring professional to join NHS Forth Valley as Dementia, Delirium Consultant Nurse/AHP.
The post-holder will provide professional nursing/AHP leadership, strategic direction, and expert consultancy on all aspects of care for people with Dementia and or Delirium across the patient pathway in NHS Forth Valley and Health & Social Care partnerships and care homes. This will involve working across professional and organisational boundaries, in partnership and in collaboration with others including service users, carers, health professionals, social work, third sector staff, care home providers and local education providers.
