Job summary

Hallmark Luxury Care Homes is seeking a Dementia Care Manager to lead the dementia community within a Hallmark home. You will train the team, ensure person-centred care, and provide support to residents and their families.

Responsibilities include promoting dementia strategy, coordinating activities, maintaining compliance with regulations, and managing the dementia budget in collaboration with the leadership team.

Qualifications

• Practical experience of working in dementia care and creating advanced care plans.
• Knowledge of care standards and the drive to meet or exceed CQC standards.
• Empathy and clear, concise communication skills for sensitive issues.
• Ability to develop meaningful activities for residents.

Responsibilities

• Residential care: promote dementia strategy and ensure relationship-centred care.
• Compliance: understand policies, ensure CQC standards are met, assist leadership with documents.
• People skills: deliver staff training, participate in conferences, coordinate activities with Lifestyles team.
• Financial management: work with leadership to manage home budget for dementia care.
• Family service: communicate with families, GPs and agencies for care reviews.
