Avery Healthcare Group is seeking a Dementia Care Manager for Barnet Grange Care Home in Barnet. You will lead the dementia community, implement the care strategy, and manage resources within budget while ensuring person-centred planning and advocacy for residents.

The role focuses on delivering compassionate, high-quality care, developing meaningful activities with the team, and promoting residents’ independence and quality of life in a supportive environment.

Formación

• Three years’ experience in a similar role in a fast-paced setting.
• Comprehensive understanding of CQC standards and requirements.
• Knowledge of care services businesses and the UK health and social care market.

We’re now looking for a warm, enthusiastic, and dedicated individual to join our award-winning team as a Dementia Care Manager at Barnet Grange Care Home in Barnet. If this sounds like the place for you!

ABOUT THE ROLE

Your focus as a Dementia Care Manager will be tobe responsible for the overall management of the Dementia Community, implementing and embedding thecompany’s dementia care strategy and managing resources within a given budget .

Other responsibilities will include:

• Ensuring that the plan care day is accurate and reflected in the care needs summary and that the resident/relative identifies needs and preferences. Ensure all information is integrated into a person-centred care plan.


• Building caring relationships with residents and families, liaising with them sensitively and appropriately. Act as an advocate for the residents' best interests, appropriately managing conflict.


• Working closely with the Activities Team to ensure the development of meaningful activities for residents with Dementia. Support and set expectations for the care team's participation in these activities.


• Promoting a culture that enables residents to maximise their capabilities, optimising their independence and enhancing their quality of life.
