Job summary

Qualifications

• Master's degree in Speech Language Pathology from a CAA accredited program.
• Must be fully licensed in Indiana to practice as a speech language pathologist; OH license if in Ohio.
• Must hold CCC awarded by ASHA or CFY under supervision; Basic Life Support required.

Responsibilities

• Evaluates patients with speech, language, cognitive, voice, and swallowing disorders.
• Re-evaluates patient status at appropriate intervals.
• Interprets evaluation data to determine rehabilitation potential and benefit from intervention.
• Identifies patient, family, and caregiver needs.
• Sets functional rehabilitation goals with measurable timeframes.
• Documents goals in functional terms with timeframes.
• Develops and implements a specific treatment plan for intervention.
• Ensures continuity of patient care by following established plans.
• Modifies care based on ongoing assessment and response to treatment.
• Provides and/or recommends equipment for home use and augmentative/alternative communication devices.
• Delivers education tailored to patient understanding and instructs on home programs.
• Collaborates with other healthcare professionals on discharge planning.
• Initiates discharge planning at initial evaluation and arranges for care items as needed.
• Supervises Clinical Fellows and SLP students on-site.

Education

Master's degree in Speech Language Pathology (CAA accredited)

CCC-SLP certification or CFY under supervision
