The Vale of Glamorgan Council seeks a dedicated professional to join the Learning Disability Day Services team. You will provide person-centred support to adults with learning disabilities, coordinate timetables of opportunity, and lead on staff supervision and performance development.
Main duties include maintaining accurate records, facilitating community engagement, and ensuring safe practices across day centres and community locations.
The Vale Learning Disability Day Service provides meaningful day opportunities to adults with a learning disability, who may also have a complex presentation of need. We are a person-centred, outcomes-led, community-based provision, which delivers day opportunities from day centre bases and in local community locations, serving citizens across the whole of the Vale. Pay Details:
Grade 6 £30,515 - £33,119 pro rata
Hours of Work/Working Pattern:
37 hours Monday to Friday
Main Place of Work:
Current base is either Woodlands or Trysor O’Le day centres but may include various locations across the Vale of Glamorgan in accordance with the needs of the role.
To work within Learning Disability Day Services providing high quality, outcome led, person centred planning and support to people with learning disabilities and complex support needs. This involves developing plans that enable outcomes to be achieved and creating timetables of opportunity for individuals, as well as coordinating teams to implement the plans and monitor their effectiveness at regular intervals.
You will provide a link worker role to allocated individuals and undertake regular reviews with the person and their networks to ensure identified outcomes are achieved, as well as supporting individuals both in community and day centre-based activities. To effectively supervise Day Service Support Workers through effective performance management. Participate in team meetings and undertake staff development and all relevant training programmes. Positively participate in your own and staff members #itsaboutme reviews. To attend to the personal and physical needs of individuals in a respectful manner and practice in accordance with agreed procedures, to include medication administration, infection control and food hygiene.
You will maintain records, to include financial records and compile reports. You will coordinate and maintain day service social media/digital platforms. To undertake effective risk management, develop safe working methods and maintain a safe working environment, to include manual handling and use of departmental vehicles. You will maintain housekeeping records and report maintenance issues to assist with the upkeep of each day centre base.
