Job summary

Hertfordshire Partnership NHS Foundation Trust is seeking a highly skilled CYP Practitioner to join the CYPMHS South Hertfordshire team. The post is full-time (37.5 hours/week) and permanent, focusing on delivering high-quality nursing input and coordinated care for young people.

Responsibilities include conducting assessments, delivering evidence-based interventions, managing risk, safeguarding, and contributing to case discussions and team meetings.

Qualifications

• CYP practitioner with relevant professional qualification and experience.
• Experience working in multidisciplinary teams.
• Knowledge of safeguarding children and vulnerable adults.

Responsibilities

• Lead assessments and deliver evidence-based interventions.
• Manage risk and safeguarding as part of care.
• Coordinate care and referrals for service users.
• Participate in case discussions, service meetings and MDTs.
• Provide supervision to colleagues and students within HPFT.
