We have an exciting role within our Adult Social Care and Health Integration as a Customer Advice and Support Worker.

We are developing ACT (Accessing Change Together), an integrated service combining council staff and partner organizations to support individuals with Housing Support, Homelessness, Domestic Abuse, and Substance misuse issues.

This initiative aims to streamline access to services, facilitate collaboration among agencies, and develop effective support pathways through multidisciplinary teams working together to assess risks and develop tailored support plans.

We are recruiting for roles within three interconnected teams: Front Door, Housing Solutions, and Community Interventions. We seek candidates passionate about making a difference, experienced in working with diverse services, and resilient in supporting individuals facing complex challenges related to Housing, Homelessness, and Domestic Abuse.
