A leading NHS Trust is seeking a Senior Mental Health Practitioner to join their dedicated team in Halifax. The role involves providing skilled interventions to patients with mental health issues, coordinating care delivery within a multi-disciplinary framework. Job summary
37.5 Hours per week
The post holder will be responsible for assessments, risk management, care-planning, and co-ordination of care delivery within a team approach.
You should be forward thinking, proactive and it is essential that you are willing to work flexibly as part of a 7-day week service, which includes shifts and working weekends and nights. The ability to form effective working relationships with fellow professionals, your team colleagues and service users is an essential requirement of the job and its a key requirement that you bring compassion to the role as the team engage with people in their daily lives.
The team provide treatment of acutely unwell patients in their own homes as an alternative to hospital admission and gatekeep service users to determine if admission is required. We also coordinate assessment and care for people detained under section 136 MHA in the locality.
A car driver and access to a car is essential to this post. A period of induction will be available for the successful candidate.

Main duties of the jobThe trust and Team value carers and the support they provide to service users and have previously been finalists in the trust excellence awards for our dedication to carers. You will be expected to embrace this approach in your work and promote the co-production of care, involving carers whenever possible and ensuring that they are supported with their role.
You would be joining a professional supportive team and benefit from structured supervision, in house reflective practice groups and special interest groups which promote new ideas and learning for the team.
Triage and assessment of referrals into the service, liaising frequently with partner agencies and wider professionals.
Deliver community and hospital based urgent assessments and intensive home based treatment.
Coordinate shifts as triage on an allocated shift basis.
Coordinate 136 referrals on an allocated shift basis.
Contribute to bed management out of working hours
In reach to acute wards to support timely safe discharges.
Delivering safe effective care regarding the teams two crisis beds.

The individual has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Advises and supports the person and carers to promote health and well-being and to prevent illness.
Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required.
Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community.
Functions as a member of a multi-disciplinary team.
