Kent and Medway NHS and Social Care Partnership Trust is seeking a Social Work Assistant to join our Dartford CRHT team. You will work under the Social Work Team and MDT guidance to support service users and carers through crisis recovery, bridging social care with clinical intervention.

Qualifications

• Hold a full, valid UK driving licence and have access to a car/vehicle for business use.
• Empathetic, resilient and highly organised.
• Experience working with service users and MDT in crisis situations or social care settings.

Responsabilités

• Liaise with a range of partnership agencies and the MDT on behalf of service users.
• Support safeguarding investigations and report safeguarding issues promptly.
• Contribute to risk assessment and risk management planning.
• Assist with capacity assessments and documentation of patient records.
• Develop skills to work with service users experiencing severe mental health problems and potential crises.
