Job summary

Kent and Medway Mental Health NHS Trust is seeking a Social Work Assistant to join the Dartford CRHT. You will work under the Social Work Team and MDT to help service users and carers navigate crisis, ensuring safe, person-centred support across community and hospital settings.

The role requires a full UK driving licence with access to a car for business use, and involves liaising with statutory and voluntary agencies, documenting care and contributing to safeguarding and risk management

Qualifications

• Must hold a full, valid UK driving licence and have access to a car for business use.

Responsibilities

• Undertake specified tasks with service users as agreed with the Social Worker/MDT.
• Liaise with partnership agencies, both statutory and voluntary, on behalf of service users and the MDT.
• Encourage feedback from service users/carers and other professionals about service delivery.
• Report safeguarding issues as a matter of urgency and complete necessary paperwork.
• Contribute to safeguarding investigations and risk assessment and risk management planning.
• Support the Child Protection policy in completing and recording contracts with SSD and Persons with PR.
• Support capacity assessments where appropriate and advised.
• Develop skills to work effectively with service users in crisis and experiencing severe mental health problems.
• Keep abreast of changes in legislation, policy and practice guidance.
