Job summary

Derbyshire Healthcare NHS Foundation Trust in Chapel-en-le-Frith is seeking a Band 5 Registered Practitioner to join a 24/7 crisis and home treatment service for adults 18–65. You will work within CRHTT and with the Community Mental Health Team to provide timely assessment, care and support in a busy NHS environment.

The post focuses on safe, high-quality care, professional supervision, and collaboration with a range of professionals. The role supports flexible working within an NHS setting.

Qualifications

• Experience of working within mental health settings.
• Ability to meet travel requirements of the post.
• Experience and skills in acute mental care or assessment services.

Responsibilities

• Contribute to 24/7 Crisis Resolution, Home Treatment Team and Community Mental Health Team activities.
• Provide clinical assessment and safe, responsive care for adults (18–65) in crisis.
• Promote evidence-based practice and work across multi-disciplinary teams to support patients and carers.
