Job summary
Derbyshire Healthcare NHS Foundation Trust is recruiting for a Band 5 Registered Practitioner to join the Crisis Resolution and Home Treatment Team and Community Mental Health Team in Buxton. You will deliver 24/7 crisis care for adults 18–65, working within a multidisciplinary service that emphasises timely assessment, evidence-based practice and patient-centred care.
The role requires registration with NMC/HCPC or equivalent, with opportunities for professional development and robust
Qualifications
- Registration with the relevant professional body (NMC/HCPC/SWE).
- Experience in mental health services.
- Ability to work within crisis response settings.
Responsibilities
- Provide 24/7 crisis and resolution home treatment for adults 18–65.
- Work within a multidisciplinary team to ensure high-quality care.
- Lead triage, assessment and timely response in CRHTT/Living Well pathways.
- Maintain professional presentation and therapeutic relationships with service users and carers.
- Collaborate with housing associations and other agencies to support social inclusion.
