Job summary

The Whittington Hospital NHS Trust is looking for a Band 3-Assistant Continence practitioner to join the Bladder & Bowel Care Service across Islington and Haringey. The role involves assessing and reviewing the care needs of patients receiving continence products, and implementing care plans through established Care Pathways to improve health and quality of life.

Qualifications

• NVQ Level 3 in care or equivalent.
• Maths and English level 1
• Completion of Care Certificate

Responsibilities

• Undertake continence assessments for new patients referred to the Bladder & Bowel Care Service with bladder and/or bowel problems.
• Perform clinical procedures as required, including bladder scanning and fitting external urinary sheaths.
• Follow up patients on agreed care pathways, monitoring progress and updating care plans as necessary.
• Identify changes in patients' conditions and elevate concerns or significant changes in care needs to the responsible nurse or physiotherapist.
• Provide continence advice and support to patients, carers, relatives, and other healthcare professionals.
• Plan and deliver care within own level of competence.
• Carry out continence assessments, reassessments, health promotion, and ongoing management within care homes, clinics, and community settings.
• Promote confidence and trust by maintaining high standards of professional conduct, appearance, and behaviour.
• Work collaboratively within the multidisciplinary team and alongside partner agencies.
• Maintain patient confidentiality in line with organisational policies and data protection requirements.
• Communicate effectively with patients, relatives, carers, and colleagues to support high-quality patient care.
• Support and participate in clinical audit and service improvement activities.
• Support the continence nursing team in delivering education and training programmes within residential and nursing home settings.
• Contribute to raising awareness and improving best practice in bladder and bowel care.
• Maintain accurate and up-to-date electronic patient records.
• Work independently, organising and prioritising workload effectively.
• Liaise with members of the wider multidisciplinary team to ensure coordinated patient care.
• Participate in clinical supervision, appraisal processes, and mandatory training.
• Undertake additional training and development opportunities identified through appraisal and personal development plans.
• Participate in clinical audits and quality improvement initiatives to enhance patient-centred care.
