ESNEFT in Colchester seeks a Consultant in Older Peoples Services (Community Frailty) to lead integrated frailty care across Northeast Essex. The role offers LTFT or full-time options with 9 or 10 PAs per week, and may be community-led or split 50:50 with acute care.

You will require specialist accreditation in Geriatric Medicine and drive CGA, risk management and home-based care when clinically appropriate. Flexible NHS benefits apply.

Qualifications

• Experience leading frailty services across integrated care models.
• Expertise in Comprehensive Geriatric Assessment (CGA) and proactive frailty management.
• Ability to manage clinical risk and support patients at home when appropriate.

Responsabilités

• Lead development of integrated frailty services across Northeast Essex.
• Provide consultant-led care across acute, community and virtual settings.
• Collaborate with multidisciplinary teams to develop care pathways and guidelines.
• Mentor and supervise junior medical staff and ensure high standards of care.
• Champion patient-centred care and promote admission avoidance strategies.
