HCRG Care Group is seeking a Consultant Geriatrician to lead our Hospital at Home service across Bath & North East Somerset, Swindon and Wiltshire. You will shape pathways, support senior decision making, and drive innovative, home-based acute care with a multidisciplinary team.

Join our innovative Hospital at Home (H@H) service and play a leading role in delivering acute hospital-level care in patient's own homes across Bath & North East Somerset, Swindon and Wiltshire. This is a unique opportunity to move beyond the traditional consultant role and influence care at a system wide level.
As the senior clinical lead for H@H, you will drive service development, provide expert frailty leadership, support complex clinical decision making, and help create new pathways that reduce hospital admissions and promote home first care. Working with a highly skilled multidisciplinary team, you will champion innovative models of care that improve outcomes and patient experience.
This role offers the chance to shape strategy, strengthen partnerships across acute, community and primary care services, and lead clinical governance, quality improvement and service transformation across the BSW system.
