CPSL Mind is hiring a Connect to Work Triage Co-ordinator in Peterborough with hybrid working. The role is 37.5 hours per week, with a salary of £27,113 per annum (April 2025). You will triage referrals, perform pre-assessments, and hand over eligible cases to specialist services, while signposting others to local providers.

Full Time Connect To Work Triage Co-ordinator

Based in Peterborough with hybrid working option

Monday to Friday, 37.5 hours per week

Salary Salary Scale point 15 £27,113.00 per annum (April 2025)

Main Objective

The Connect to Work Triage Coordinator is a key role, coordinating activity between referrers and partner providers to ensure individuals receive support and guidance to support them to enter, return to, or stay in work. This role is responsible for triaging Expressions of Interest for the Connect to Work programme, delivered in partnership with Cambridgeshire County Council and other specialist providers. This role will:

  • Provide the first point of contact and support for individuals referring in
  • Oversee completion of pre-assessment of eligibility against defined criteria
  • Ensure the smooth handover of eligible referrals through to specialist employment services
  • Connect ineligible referrals with an appropriate local service, maintaining a strong understanding of CPSL Mind services, local groups and other local service providers
  • Support the collection of data and impact stories required for reporting against key performance indicators.

Specific Duties and Responsibilities
Initial Assessment and Engagement
  • Contact individuals who have been referred to Connect to Work within prescribed timeframe, to carry out an initial pre-assessment against criteria.
  • For those referred who do not meet eligibility for Connect to Work, provide advice, information and signposting to an appropriate local service, and recording this activity appropriately. Informing the referrer as to why the referral has been rejected.
  • For those who do meet eligibility criteria, complete the introduction to specialist employment service, ensuring a smooth handover
  • Provide check in calls for people who have completed their plan.
Relationships
  • Maintain a strong knowledge of CPSL Mind services and other local service providers and community groups
  • Maximization of referrals in, maintaining strong communication and trusted relationships with referrers and local organisations
  • Act as first point of contact for Connect to Work enquiries
  • Work closely with broader CPSL Mind teams, including the Economic Inclusion team
Monitoring and Evaluation
  • Support tracking of individuals progress toward goals
  • Maintain accurate and up to date records
  • Monitor data across the team to support evidence of key performance indicators being met.
Compliance and Best Practice
  • Support the team to stay informed about relevant legislation, policies, and best practice related to disability employment and the workplace.
  • Ensure all activities comply with legal and organisational standards.
PERSON SPECIFICATION
ESSENTIAL

Empathy and Understanding: Ability to connect with individuals and staff with empathy, and understanding in relation to their unique challenges and needs.

Communication: Strong verbal and written communications skills, with the ability to clearly articulate ideas and instructions.

Problem Solving: Proactive approach to identifying solutions and overcoming barriers.

Qualification: Mathematics and English (Minimum GCSE level 4 or equivalent)

Excellence in the use of Microsoft 365 applications including, Sharepoint, Excel and Word.

DESIRABLE

Experience of working in a front facing environment.

Understanding of local area / providers / groups and organisations

Understanding of employment support and welfare/benefits

Understanding of risk management and protection of vulnerable adults.
