Job summary

Acuity INC is seeking a qualified nurse to lead and mobilise high-standard care packages in the Southampton area. You will perform clinical assessments, develop care plans, and mentor staff to ensure adherence to specialist interventions.

The role spans adult and paediatric packages, with strong emphasis on documentation and auditing to meet CQC expectations. You will be responsible for coordinating care plans, risk assessments, and ensuring smooth operations within multidisciplinary teams.

Qualifications

• Qualified Nurse or RGN with Complex Care or Learning Disabilities experience.
• Strong communication and organisational skills.
• Competent in maintaining and reviewing clinical documentation.

Responsibilities

• Leading the mobilisation of new care packages, including clinical assessments and care planning
• Training and mentoring care staff, ensuring competence in specialist clinical interventions
• Developing and reviewing care plans and risk assessments
• Monitoring and auditing care delivery to the highest standards
• Building relationships with clients, families and multidisciplinary teams
• Providing leadership and supporting investigations when required
• Caseload includes adult and paediatric packages
• Tracheostomy and ventilator care competency; DBS funded

Job description
