Job summary

A local NHS Trust in Enfield is seeking a Band 7 Complex Mental Health Discharge Coordinator to support complex discharge arrangements and liaise with multidisciplinary teams. This is a temporary role with a competitive pay rate of £30 per hour, working Monday to Friday.

We are working with a local NHS Trust who are seeking a Band 7 Complex Mental Health Discharge Coordinator to join their service in Enfield.

You'll be responsible for supporting the ward staff with complex discharges, taking the lead in discharge arrangements for individual patients where appropriate, liaising effectively with all members of the Multi-Disciplinary Teams in Community settings.

What's on offer:

Location: Based at a Hospital in Enfield

Rate of pay: £30 p/h Umbrella (potentially negotiable depending on experience)

Working Hours: Monday-Friday, 9am-5pm

Contract: 3 month initial contract with the view of extension

Requirements:

  • RMN/Social Worker with a valid registration
  • Experience of the CHC process
