Job summary

Claimont Health is seeking a Complex Care Support Worker to join its clinical team. The role provides one-to-one care in clients' homes, either live-in or home-visit, supporting individuals with complex needs and CAMHS experience.

You will deliver person-centred care, maintain records within 24 hours, and work with families and carers to promote mental health, safeguarding, and community participation. A UK driving licence is required and enhanced DBS disclosure is expected.

Qualifications

• Basic computer skills
• 12 months' experience as a complex care support worker
• CAMHS experience
• Working collaboratively in a multi-professional team
• Evidence of appropriate resource management

Responsibilities

• Provide one-to-one care to clients in their homes on a live-in or home-visit basis
• Support clients in community-based activities for personal development and progression
• Deliver person-centred, evidence-based care and maintain core psychiatric skills
• Work with families and carers to help them understand mental health and cope with caring
• Keep written and electronic patient records, completed within 24 hours of events
• Maintain client confidentiality and comply with data protection and information governance rules
• Communicate with referring agents, colleagues, carers, relatives and community services
• Attend and fully contribute to team meetings
• Report assessment findings in the appropriate format
• Safeguard children and adults, follow safeguarding procedures and raise concerns
• Report accidents, incidents and near misses, and manage health and safety
• Complete mandatory training and take responsibility for own professional development and performance review
