Job summary

Clover care group limited is seeking a compassionate Domiciliary Support Worker to provide specialised assistance to a young male service user with complex needs, including Duchenne muscular dystrophy. Shifts run daily with early, late and overnight blocks; you will work alongside a second carer to deliver personalised care plans, ensuring safety, independence and well-being.

The role involves driving to appointments, recording updates, and collaborating with family and healthcare professionals

Qualifications

• Experience with complex care, especially Duchenne muscular dystrophy.
• Ability to develop or follow personalised care plans.
• Strong communication with families, teams and external professionals.
• IT literacy for accurate documentation and reporting.
• Valid driving licence preferred.

Responsibilities

• Deliver personalised care per care plans, focusing on service user's needs.
• Assist with activities of daily living including personal hygiene, feeding and mobility.
• Support during outings and community activities; ensure safety and engagement.
• Maintain accurate records of care and changes in condition.
• Communicate with families and healthcare teams to coordinate care and updates.
• Drive to appointments, activities or classes ensuring safe transportation.
• Move and handling as required and adherence to health and safety protocols.
