Job summary

Kent Community Health NHS Foundation Trust in Hawkhurst is seeking a Complex Care Nurse (NHS AfC: Band 5) for a 30-hour part-time contract at Hawkhurst Community Hospital. You will work within integrated neighbourhood teams, delivering joined-up care for people with frailty and long-term conditions at home.

The role offers development opportunities, flexible working, and a focus on reducing hospital admissions, with access to training and supervision to grow your clinical skills.

Qualifications

  • Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
  • Recent healthcare experience gained through education, work experience or previous employment.
  • You will be a registered health care professional with the Nursing and Midwifery Council.
  • A full, valid UK driving licence or a full driving licence issued by an EU country

Responsibilities

  • Phlebotomy, blood pressure and weight monitoring.
  • Continuous Glucose Monitoring (CGM) set-up and review.
  • Foot and skin assessments and administering medications including insulin.
  • Providing education to promote self-management and independence.

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Main duties of the job

Are you passionate about making a real difference to people living with frailty and complex long-term conditions? Join our Complex Care Nursing team and help enable people to live well at home and reduces hospital admissions.

Working across a designated Primary Care Network, you will support a caseload alongside experienced Band 6 and Band 7 Complex Care Nurses. You'll undertake routine assessments, patient reviews and monitoring, supporting people with conditions such as Heart Failure, COPD and diabetes.

Key responsibilities include phlebotomy, blood pressure and weight monitoring, Continuous Glucose Monitoring (CGM) set-up and review, foot and skin assessments, administering medications including insulin, and providing education to promote self-management and independence.

Qualifications, training and experience
  • Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
  • You’ll need to have recent experience of healthcare gained through education, work experience or previous employment
  • You will be a registered health care professional with the Nursing and Midwifery Council
  • A full, valid UK driving licence or a full driving licence issued by an EU country
Knowledge and skills
  • You will need to have a customer focussed approach which is essential to: Provide patients, clients and their families with the best possible experience
  • You will need to be able to be able to use IT systems and databases
  • You will understand where to signpost clients and families to alternative services to support behaviour change and improve health
  • You will know when you are within your own limits and when to escalation
  • You will have knowledge of up-to-date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker
  • Having an awareness of developments within the NHS would be an advantage
