An exciting opportunity has arisen for a Band 6 Community Mental Health Nurse Specialist to join the Older Adult Community Mental Health and Dementia Nursing Team.
You will work as a Nurse Practitioner within a multidisciplinary team, providing assessment, treatment, and support for individuals experiencing mental health difficulties, including those undergoing assessment and diagnosis for dementia.
We are seeking a dynamic and experienced Mental Health Practitioner who is adaptable, motivated, and able to work both independently and as part of a multidisciplinary team.
The successful candidate will have experience in undertaking comprehensive assessments, including dementia reviews, physical healthcare assessments, health promotion, social inclusion, and recovery-focused interventions.
The post holder will be responsible for supporting, coordinating, and reviewing the care needs of individuals with mental health conditions and dementia within the Older Adult Community Mental Health Team.
A key aspect of the role will involve supporting patients in their own homes, working closely with families and carers, facilitating hospital discharge, and collaborating effectively with a range of partner agencies and healthcare services to ensure seamless, high-quality care.
You will have experience of multi-agency working, be able to prioritise a demanding workload effectively, and demonstrate strong problem‑solving skills and the ability to manage the challenges of day-to-day clinical practice.

Work that wraps around your needs
To be responsible as the Care Coordinator for assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers within a defined catchment area/locality. Provide professional advice and support to other agencies and members of the MDT.
To be responsible for ensuring that there is clear communication with clients and carers, actively listening to establish an understanding of the needs of the service user and their carers.
To be responsible for coordinating the care and and assessment under the Care Programme Approach of a designated caseload
Responsible for ensuring that the requirements of the Mental Health Act 1983 are observed, adhered to and implemented.
As a newly appointed employee, you are responsible for incurring the cost of your initial DBS check relevant to your post; the amount will be deducted from your first salary with the Trust.
