• Encourages client independence while offering hands-on support.
• Follows care, support and therapy plans; reports changes to Case Manager.

Responsibilities

• Follow risk management plans and inform Case Manager of concerns.
• Communicate effectively and positively with client and team.
• Complete daily notes and necessary recordings.
• Attend training and team meetings as arranged.
• Adhere to policy, procedures and approved team policies.
