Job summary

Buckinghamshire Mind is seeking 2 x Community Connectors to join our Berkshire-area team. Two permanent, 37-hour roles based in Slough and Windsor, Ascot & Maidenhead, with a salary of £25,250 per year.

You will support patients to access community services, work in primary care, and coordinate with agencies to ensure ongoing wellbeing. A driving licence and vehicle access are preferred for community travel.

Qualifications

• Holds a relevant degree, diploma, or equivalent experience.
• Self-sufficient, well-organised and IT-literate.
• Empathetic and culturally aware with strong communication.
• Able to collaborate with diverse teams and promote service use.

Responsibilities

• Work in a multi-disciplinary team in primary care.
• Assist assessments, support risk assessments and set goals.
• Provide self-management tools and support personal plans.
• Build relationships with agencies and healthcare professionals.
