Job summary

Thistle is seeking Health & Wellbeing Practitioners to join our Wellbeing teams in Midlothian and East Lothian GP practices. You will work with a team to support people living with long-term conditions to self-manage, traveling across Midlothian or East Lothian to engage referred clients.

The role involves persistent engagement, caseload management, and co-delivery of cancer journey contracts, using strengths-based outcomes to link people to community supports and self-management coaching.

Responsibilities

• Work with a team of practitioners to deliver our Midlothian or East Lothian GP contract
• Be prepared to travel across Midlothian or East Lothian to engage with people referred to the Wellbeing Service (see location)
• Practice persistent engagement to help people who struggle to receive support
• Manage a caseload of people referred to the service
• Co-deliver the Macmillan, Improving the Cancer Journey contract (in Midlothian)
• Use a strengths based personal outcomes approach to help people identify and access appropriate support for them to self-manage including: Providing specific coaching in self-management; Delivering group based Lifestyle Management courses; Linking people to community resources and supports; Support volunteers who have lived experience of long-term conditions into peer roles
