Job summary

Sirona care & health in Bristol, North Somerset or South Gloucestershire seeks a Support Worker to join a multidisciplinary team delivering therapy-based support in community settings. You will work alongside the community nursing team to help patients regain independence at home and improve quality of life.

We look for compassionate, patient individuals with strong communication skills who can work independently and as part of the MDT.

Qualifications

• NVQ Level 3 in Health & Social Care or willingness to work towards.

Responsibilities

• Deliver therapy-based support under guidance of health care professionals.
• Help patients develop daily living skills and independence.
• Assist in goal setting and motivate progress.
• Monitor progress and report to MDT and lead professionals.
• Promote safe, supportive environments and patient participation.
• Collaborate with physiotherapists, occupational therapists and nurses.
• Encourage community engagement for patients.
