Job summary

Norfolk and Suffolk NHS Foundation Trust seeks a Community Team Manager for Older Persons services. You will lead a multidisciplinary team in a community setting, ensuring safe, effective care and strong service development.

Qualifications

• Ongoing professional development toward degree level

This post includes our Community Mental Health Team (CMHT), our Intensive Older Person Service (IOPS) and also supports the Memory Assessment and Treatment Service (MATS)

Main duties of the job

In this role, you will provide strong clinical and operational leadership to a multidisciplinary community team, ensuring the delivery of safe, effective, and high-quality care to older adults. You will be responsible for overseeing day-to-day service delivery, managing performance, and ensuring that care is responsive, evidence-based, and aligned with national and local standards.

You will lead, support, and develop a diverse team of practitioners, fostering a positive and inclusive culture that promotes staff wellbeing, continuous learning, and professional development. This includes supervising staff, managing caseloads, and ensuring effective risk management and safeguarding practices are embedded across the service.

Working collaboratively with internal and external stakeholders, including education, social care, and third-sector partners, you will contribute to integrated care planning and service improvement initiatives. You will also play a key role in strategic planning, implementing service developments, and ensuring the efficient use of resources to meet demand.

Additionally, you will monitor and report on key performance indicators, support quality assurance processes, and drive improvements to enhance patient outcomes and experience. Your leadership will ensure that the service remains responsive to the needs of the local population while maintaining compliance with regulatory and organisational requirements.
