Job summary

NSFT is recruiting a Community Team Manager for the Older Persons service in Norfolk and Suffolk. You will provide strong clinical and operational leadership to a multidisciplinary team, ensuring safe, effective, and high-quality care for older adults.

Job description

Are you an experienced manager or leader within a community mental health setting. Do you have experience or an interest in working with Older Adults? Then we want to hear from you.

We are looking for passionate, skilled professionals from a Nursing, Social Work (with additional mental health training), or Occupational Therapy background.

We are excited to be recruiting a Community Team Manager to join our Older Persons service at a time of genuine transformation and innovation.

Our service is evolving as part of a wider transformation agenda. This creates a unique opportunity to be part of shaping a modern, responsive mental health service for Older Adults. This post includes our Community Mental Health Team (CMHT), our Intensive Older Person Service (IOPS) and also supports the Memory Assessment and Treatment Service (MATS)

In this role, you will provide strong clinical and operational leadership to a multidisciplinary community team, ensuring the delivery of safe, effective, and high-quality care to older adults. You will be responsible for overseeing day-to-day service delivery, managing performance, and ensuring that care is responsive, evidence-based, and aligned with national and local standards.

You will lead, support, and develop a diverse team of practitioners, fostering a positive and inclusive culture that promotes staff wellbeing, continuous learning, and professional development. This includes supervising staff, managing caseloads, and ensuring effective risk management and safeguarding practices are embedded across the service.

Working collaboratively with internal and external stakeholders, including education, social care, and third-sector partners, you will contribute to integrated care planning and service improvement initiatives. You will also play a key role in strategic planning, implementing service developments, and ensuring the efficient use of resources to meet demand.

Additionally, you will monitor and report on key performance indicators, support quality assurance processes, and drive improvements to enhance patient outcomes and experience. Your leadership will ensure that the service remains responsive to the needs of the local population while maintaining compliance with regulatory and organisational requirements.
