Job summary

Qualifications

• Hold a Level 2 in Maths and English.
• Experience of working with adults in a social care setting.
• Ability to travel across Leicestershire for visits.
• Understand need and risk assessments and related issues.

Responsibilities

• Meet individuals, families and carers to assess needs under the Care Act 2014.
• Devise access plans for required support, including community connections or referrals.
• Identify safeguarding concerns and consider mental capacity and high-risk situations.
• Undertake learning, induction and progression toward Grade 9 responsibilities (assessment, support planning, case management).
