South Warwickshire University Foundation Trust Community Services is recruiting a Band 3 Community Support Worker to join the Community Response Team. You will provide compassionate personal care to patients at home under supervision and help prevent avoidable hospital admissions.

The role covers South Warwickshire with bases in Stratford-upon-Avon and Leamington Spa, requiring a driving licence and access to your own car.

Qualifications

  • GCSE English and Maths (A-C) or equivalent.
  • NVQ/Diploma Level 2, willingness to progress to Level 3 in healthcare.
  • Experience delivering care in healthcare or people-centred settings.
  • Good written, verbal and non-verbal communication.
  • Eligible to drive with access to own car.

Responsibilities

  • Provide high-quality, compassionate personal care under supervision.
  • Support patients to stay independent at home and assist with safe discharges.
  • Work as part of a multidisciplinary team with social services and GP practices.
  • Maintain accurate patient notes with clear documentation.

Job description

An exciting opportunity has arisen for an enthusiastic Band 3 Community Support Worker to come and join us in our Community Response Team. Are you interested in working within South Warwickshire University Foundation Trust Community Services? Are you a carer, a support worker, or someone who has always been interested in working for the NHS? If so, we would love to hear from you! Our Community Response Team covers South Warwickshire, has bases at Stratford upon Avon and Leamington Spa working across the county supporting patients that have identified health and social needs as a result of a change in their health condition. The service is committed to provide high quality care to provide an immediate response, aiming to prevent avoidable hospital admissions by providing support to patients to remain independent within their own home and facilitate safe discharges home from hospital. The team works together to support patients in the community and work closely with social services, GPs, community nursing teams, patients, relatives and carers. To be successful in this role you must hold a valid driving licence and have access to your own car. The Community Response Team operates across the hours of 08:30-21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated pro rata, we offer a mix of early, late and long days. The Band 3 Clinical Support Worker will undertake high quality, caring and compassionate personal care and associated duties under the direction but not direct supervision of the registered staff and working as part of the multidisciplinary team. As a Band 3 Clinical Support Worker (CSW) you make a valuable and important contribution to the delivery of high quality healthcare, supporting the registered professional in the delivery of safe, effective, compassionate patient care.   • Obtained an educational level of GCSE level A - C English and Maths or equivalent
  • Minimum NVQ/Diploma Level 2, willing to work to Level 3 or equivalent higher apprenticeship in healthcare, NVQ /Diploma level 3 or equivalent higher apprenticeship in healthcare, Experience in delivering care in a healthcare or people centred environment
  • Experience working as part of a team, Able to delegate work appropriately
  • Good written, verbal and non-verbal communication skills
  • Competent IT skills
  • Able to present information, written and orally in an accurate clear and logical manner
  • Ability to organise and prioritise workload
  • Ability to write accurate and legibly in patient notes, Able to work as part of a team

  • Able to work flexibly over a 24 hour period
