Job summary

Sirona care & health is hiring a Community Support Worker to help adults live independently in Bristol, North Somerset or South Gloucestershire. You will work with clinicians to provide nursing tasks, rehabilitation and daily personal care in the community.

Full and part-time hours are available with flexible patterns. Enhanced pay for weekends and bank holidays, mileage reimbursed, NHS pension and generous annual leave. Training tailored to your development with progression opportunities.

Qualifications

• Experience of direct patient care in a health or social care setting.
• Ability to work as part of a multidisciplinary team.
• Comfort with travelling within the service delivery area and using IT systems such as email and EMIS.

Responsibilities

• Undertake delegated clinical, therapy and nursing activities to support service users.
• Provide safe, patient-centred clinical activities in the community.
• Deliver a full range of healthcare activities within adults services.
• Be flexible to work with other teams or services within locality.
• Be willing to travel throughout the service area.

Would you like to make a difference to the community you live in and love? Are you looking to start a career in healthcare or are currently a Healthcare Assistant or Support Worker looking for a fresh start? Our teams support patients to live independently and improve their quality of life in the place they like to call home. As a Community Support Worker, you will play a vital role in our service. You will work closely with the multi-disciplinary team made up of clinicians such as Physiotherapists and Nurses to provide ongoing clinical activities such as, nursing tasks, rehabilitation, recovery and support with daily routines and personal care.

Responsibilities

• Undertake a range of delegated clinical, therapy & nursing led activities to support the service users on the Integrated Network Team caseload.
• To provide a safe, patient centred and effective clinical activities in the community setting.
• You will work within adults services to deliver a full range of healthcare activities.
• You may be expected to work flexibly with other teams or services, both within the locality and Sirona wide when the need arises at short notice.
• Be able and willing to travel throughout the service delivery geographical area.
• Explain how you've worked without direct supervision, to achieve the objectives of a post.
• Tell us about your experience of direct patient care in a health or social care setting.
• Tell us about your experience of team working and an understanding of team dynamics including what factors make a team work well and what can go wrong and why.
• Able to use a variety of IT systems e.g. email, EMIS.
• Recent relevant experience within a community setting.

Enhanced pay for weekends and bank holidays, mileage is reimbursed whilst you carry out your duties at work. We also offer generous annual leave allowance and an attractive staff benefits scheme. We have full and part time hours available and can support flexible working patterns - currently this service runs from 8am - 8pm - 7 days a week.
