Are you passionate about supporting others to live independent, fulfilling lives? Mid Norfolk Mencap is looking for a dedicated Community Support Worker to join our Outreach team. In this full-time role, you will provide one-to-one support for adults with learning disabilities, helping them to thrive in their homes and local communities.
As a Community Support Worker, you will:
Provide tailored 1:1 support at home and in the community
Help individuals build independence through support with daily living tasks such as shopping, cooking, managing money, and accessing health or leisure services
Encourage social inclusion and support people to try new experiences
Work alongside families, other professionals, and team members to ensure truly person-centred care
Support individuals to make informed choices, while respecting their dignity, rights, and privacy
Maintain accurate records and follow care and support plans
Monitor wellbeing and report any changes to the management team
Be a key contact for individuals you support, helping to coordinate their care and maintain updated support plans

A driver with access to your own vehicle this is essential
