Job summary

Essex County Council is seeking a Community Support Worker to work alongside qualified social workers and occupational therapists, delivering targeted interventions for vulnerable adults. You will implement care plans, provide practical advice and support, and help people live healthier and more independent lives.

Applicants with activity-based experience in social care, a Level 3 qualification, and a commitment to partnership working will fit well.

Qualifications

Experience within a social care setting is desirable.
Evidence of continuing professional development in relevant area.
Experience of building strong relationships with adults, families and carers in challenging times.
Experience of direct activity-based work with adults.
Knowledge of social care policy guidance and legislation.

Responsibilities

Supports social workers and occupational therapists to implement and monitor care plans.
Maintains high standards of professional practice and contributes to service improvement.
Highlights safeguarding issues and implements protective plans for vulnerable adults.
Builds relationships with clients, families and carers to provide practical advice and support.
Collaborates with internal and external partners to achieve better outcomes.
Provides information on accessing third-party services relevant to client needs.
