Working alongside experienced nurses and multidisciplinary colleagues, you will provide high-quality nursing care, undertake assessments and care planning, deliver evidence-based interventions and support patients to achieve the best possible outcomes in their preferred environment.
This is a fantastic opportunity to build your community nursing skills whilst working within a team committed to continuous improvement, professional development and delivering outstanding patient care. You'll have the autonomy to manage your own workload whilst benefiting from clinical supervision, mentorship and support from experienced colleagues.
As a Band 5 Community Registered Nurse you will be part of our valued team, receiving access to exclusive rewards and benefits including:
You will provide nursing care in accordance with national and local policies, professional standards and the Nursing and Midwifery Council Code. Working closely with the wider multidisciplinary team, you will contribute to patient assessments, care planning, implementation of treatment plans and ongoing evaluation of care to ensure patients receive safe, effective and person-centred support.

You will actively support health promotion initiatives, provide advice and education to patients and carers, and contribute to service developments that improve patient outcomes across the community nursing service. You'll also participate in quality improvement activities, audits and service initiatives designed to enhance patient care.
You will maintain your professional registration, participate in regular clinical supervision, complete mandatory training and contribute to the learning and development of others. Opportunities may also be available to support students and develop mentorship skills as part of your professional growth.
