Job summary

Sussex Community NHS Foundation Trust is seeking a registered nurse to join the Crawley Primary Care Network (PCN) Team serving Crawley and Horley. The role focuses on diabetes, wound care (including leg ulcers), palliative and continence care, and community-based support for rehabilitation and self-management.

Applicants will benefit from an induction plan and opportunities for training, development, and research within a supportive network across Sussex.

Qualifications

• Registered nurse with current registration
• Ability to work in community settings
• Willingness to participate in induction and training programs

Responsibilities

• Diabetic management and wound care including leg ulcers diagnosis and treatment
• Palliative care and continence care (bladder and bowel) including catheters
• Support rehabilitation, self-management and independence in the community
• Coordinate care with Crawley PCN team and partner providers

Job description

The main aspects of the role are diabetic management, wound management including leg ulcers diagnosis and treatment and palliative care. Also bladder and bowel care including urethral and supra pubic catheters and other continence needs.

As well as the traditional type of patients as stated above, we work in partnership with the local health and social providers covering a specific group of people under frailty.

Our goal is to deliver high quality care in the community, working with patients towards rehabilitation, self-management and independence throughout the continuum of care. To participate in a team of colleagues supporting evidence based care, facilitating staff learning and development.

The Crawley Primary Care Network (PCN) Team is looking for a registered nurse to join the team. We are based at Crawley Hospital and cover Crawley and Horley.
