Job summary

Lincolnshire Community Health Service (LCHS) has an opportunity for Registered Nurses to join their South Lincs Community Nursing Team, with 3.45 full-time positions. The role involves delivering care to patients in their homes, care homes and clinics.

You will assess needs, plan, deliver and evaluate nursing care using evidence-based practice, working autonomously and with an MDT to coordinate patient care.

Job description

Lincolnshire Community Health Service (LCHS) have an opportunity for Registered Nurses to join their South Lincs Community Nursing Team.

We have vacancies for 3.45 full time position.

As a community staff nurse you will be required to work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will be responsible for assessing care needs, and the planning, delivery and evaluation of nursing care using innovative evidence based practice.

Community nursing is exceptionally rewarding as it allows wonderful therapeutic relationships to be developed with patients in their own homes. Working in this capacity embodies the person centred care approach and allows patients to truly be at the centre of care delivery.

The skillset of a community nurse is vast and non-exhaustive requiring knowledge and expertise in a range of areas. Examples of interventions provided are wound management, continence care, palliative care, medication administration and long term condition management. Close working with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patients care is effectively co-ordinated, demonstrates a positive impact on our patients.
