Job summary

Lincolnshire Community Health Services NHS Trust in Stamford invites applications for a Senior Practitioner in Out of Hours care. The role focuses on delivering face-to-face consultations and telephone triage during the 19:00–22:00 slot on weekdays with extended hours on weekends and bank holidays.

Successful candidates will have minor illness expertise, GP practice or ACPS work experience, and a non-medical prescribing qualification or be willing to work towards it.

Job overview

We are looking for a Senior Practitioner to work within Stamford Out of Hours delivering face to face consultations and telephone triage.

We are looking to recruit dynamic individuals with minor illness skills that will also compliment the existing team of GP's, ACPS and HCSW's.

This is a part time post 14 hours a week. We are looking to employ dynamic individuals with recognised qualifications and experience in minor illness and or experience working in GP practice, which will also compliment our existing team of GPs, Advance Care Practitioner and Health Care Support Worker's.

Main duties of the job

Main duties will be to deliver a service 19:00pm-22:00pm Monday- Friday, weekends and bank holidays are 08:00am- 22:00pm to the community. The service will include see, treat and management of patients with minor illness.

See, treat, manage , discharge and or refer patients of all age ranges to other LCHS services or partner services across Lincolnshire

On occasion you may be required to support other urgent care services such as UTC's, home visiting and the Clinical Assessment Service (CAS) which run 24/7 services. There is daily GP support available remotely or in person as well as well-established referral pathways into specialities

See, treat, manage , discharge and or refer patients of all age ranges to other LCHS services or partner services across Lincolnshire.

Successful candidates will need to demonstrate that they are able to meet the essential criteria in the person specification and can provide evidence of experience in the treatment of minor illness. It would be essential for candidates to hold a non-medical prescribing qualification or willingness to work towards.

• Experience of leading evidenced service improvement and innovation through service/practice development initiatives, audit or research.
