Job description

Spinnaker Ward located at St Mary's Hospital Portsmouth is looking to recruit a compassionate and motivated Registered Nurse to join our dedicated inpatient team.

This is an excellent opportunity for a Registered Nurse to play a key role in delivering high-quality physical healthcare within an inpatient setting. You will be responsible for assessing, planning, implementing and evaluating care, ensuring patients receive safe, effective and person-centred nursing interventions.

The role includes undertaking physical health assessments, monitoring patients' physical wellbeing, recognising and responding to clinical deterioration, administering medications, supporting the management of long-term conditions and working collaboratively with multidisciplinary colleagues to achieve positive patient outcomes.

We are looking for an enthusiastic nurse with strong clinical assessment skills, excellent communication abilities and a commitment to delivering outstanding patient care. Whether you are newly qualified or an experienced nurse seeking a new challenge, you will be supported through induction, supervision and ongoing professional development opportunities., Working within professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.

The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.
