We offer developmental and training opportunities and are seeking highly motivated, passionate Band 5 Community Nurses keen to develop into roles such as:

• Band 6 District Nurse by undertaking the District Nurse Qualification.
• Chronic Condition Case Manager through disease-specific training to provide evidence-based care for individuals with complex needs.
• Nurse Practitioners by completing a modular approach to attain clinical skills.

Main duties of the job

The successful applicant will need excellent communication skills and the ability to deliver clinically effective care within the District Nurse Team. The Central Division of Betsi Cadwaladr University Health Board provides District Nursing Services across Conwy and Denbighshire, working with partner organizations to deliver high-quality, evidence-based care to help patients remain in their homes and communities, reduce hospital admissions, empower self-management, and ensure seamless, patient-centered care.

You must be able to travel to patients' homes in both urban and rural areas promptly. This post operates on a rotational 24-hour basis.

Proficiency in Welsh is desirable; applications from Welsh and English speakers are equally welcome.
