Job description

This is an ideal opportunity for a Registered nurse who is seeking to broaden and utilise their range of skills and Knowledge to nurse people in their own environment as part of a multi-disciplinary team. The Community Staff Nurse is responsible for the delivery of prescribed nursing care to patients on a day to day basis. This would include the holistic assessment of patient care needs and the development, implementation and evaluation of evidence-based care. Duties include palliative care support and assessment ,care at the end of life , supporting and assessing individuals and carers needs with long term health conditions, promoting of self -management skills and wound care The post holder will be autonomous in their clinical practice whileacknowledging their limitations and actively seek advice as necessary. The Machynlleth District Nursing Team are an integrated part of the virtual ward that aims to prevent unnecessary admissions to hospital and support early discharge from hospital .

Main duties of the job

Responsible for providing support to the District Nursing Team in the day to day management of multidisciplinary/multi-agency packages of scheduled and unscheduled care for patients with multiple, complex and palliative care needs. This will ensure that an efficient and effective service is delivered to the practice population. Responsible for prioritising their own delegated home visits according to the patients' clinical need and geographical location.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
