Job summary

Buckinghamshire Council is seeking a Social Work Assistant for the Home Independence Team to support adults in maintaining independence and wellbeing across Buckinghamshire. The role involves strengths-based assessments, care planning, and collaborative work with families and health partners.

You will work in a mobile, flexible environment with alternate weekend shifts and a 7:00am–10:00pm pattern, contributing to person-centred outcomes and safe practice.

Qualifications

• Experience in Health and Social Care or a related setting.
• Ability to undertake strengths-based assessments and develop care plans.
• Knowledge of safeguarding, Care Act and promoting wellbeing.

Responsibilities

• Undertake strengths-based assessments and reviews using the Better lives framework.
• Develop and review person-centred care and support plans.
• Support individuals to achieve identified goals and outcomes.
• Carry out risk assessments and promote safe practice.
• Collaborate with families, carers, health colleagues and partner agencies.
• Monitor and review care arrangements for effectiveness.
• Maintain accurate records and documentation.
